Maintain a log of 20 practicum hours as part of the required 100 clinical hours, contingent on approval from the supervisor. This record should document patient progress and challenges encountered during the practicum experience.

Action item: Take the preceptor’s feedback and approval and start data collection. Firstly, patient consent forms must be obtained for data collection. Secondly, coordinate with clinic staff to access relevant patient records. Thirdly, develop standardized documentation templates for tracking dietary, exercise, and stress management data. Fourthly, schedule time at RCH to collect initial and follow-up HbA1c readings. Finally, clinic policies on patient privacy and data sharing should be reviewed.

Action item: Develop a comprehensive lifestyle modification program incorporating evidence-based diet, exercise, and stress management strategies. Collaborate with healthcare providers at RCH to recruit eligible adults with type 2 diabetes (ages 18-65). Obtain patient consent and baseline HbA1c levels for comparison. Implement the program, ensuring patient adherence through regular follow-ups. Monitor progress monthly, collect post-intervention HbA1c levels after six months, and evaluate the program’s effectiveness against standard care outcomes.

Clinical Hours

Clinical hours will focus on implementing the lifestyle modification program, including patient education, tracking dietary, exercise, and stress management adherence, and collecting pre- and post-intervention HbA1c data. Review

A systematic and critical analysis of the available literature on the comprehensive lifestyle modification programs in type II diabetes. An emphasis on research in dietary behaviors, exercise, and stress reduction on glycemic control, HbA1c in particular. The efficacy of these interventions in comparison with usual diabetes care should be discussed in terms of outcomes including better glycaemic control, better compliance with diet and exercise, and improved quality of life. Assess the intervention designs, patient involvement approaches and addition sustainability.

Action item: 

  1. Search for latest peer-reviewed articles (five years old), with prime focus on lifestyle intervention programs for consumers with type 2 diabetes.
  2. Review evidence related to effects of diet, physical activity and stress reduction on HbA1c.
  3. Summarize key findings.
  4. Identify and emphasize intervention approaches that have a possibility of being implemented in the future.

Stakeholder Involvement

Involve nurses, physicians, dietitians, fitness trainers, and mental health counselors. Type 2 diabetes patients will be involved in the intervention. Administrative staff will help in data collection while leadership will ensure that data collection is in agreement with the organizational objectives. There will be feedback from the stakeholders during the implementation and the evaluation process..
